Your pet’s safety and comfort are our first priority. All clients must agree to each of the following items prior
to their pet being groomed by Carolina K9s. This agreement will remain in effect for the lifetime of the client - groomer relationship, and is applicable to all current and future pets under owner’s care unless an update occurs with new signatures.
Severe Matting
Occasionally, pets will arrive to their grooming appointment with severely matted hair. Mats in a pet’s coat grow tight, and can ultimately damage and tear the pet’s skin, which provides a breeding ground for bacteria and parasites. Removing severe mats by brushing is torture to your pet. Therefore, a complete shave-down or shorter haircut may be necessary.
Furthermore, removing a heavily matted coat can cause nicks, cuts, or abrasions due to skin growths trapped in the mats. Heavy matting can also trap moisture and urine near the pet’s skin allowing mold, fungus or bacteria to grow, producing skin irritations that existed prior to the grooming process. After- effects of mat removal procedures can include itchiness, skin redness, self-inflicted irritations or abrasions, and failure of the hair to regrow. In some cases, pets may also exhibit brief behavioral changes.
Prevention is the best defense by scheduling regular grooming appointments every 4-6 weeks, as well as regular brushing at home. If your pet needs to be shaved to remove matting, by signing this form, you acknowledge that you agree to this procedure and any risk. We will contact you prior to the shave down and you are welcome to take your pet
elsewhere if you do not wish for them to be shaved by Carolina K9s. However, the one hour minimum charge will apply, even if we do not groom your pet.
Accidents and Medical Emergencies
Although accidents and medical emergencies are very rare, there is a risk when dealing with pets. Grooming equipment is sharp, and although we use extreme caution and care in all situations, possible problems could occur including cuts, nicks, scratches, quicking of nails, etc. In most cases this can happen when a pet is wiggling or moving around and is
usually minor. In the event an accident does occur, you will be notified of the accident. If Carolina K9s feels it is serious, and the owner is not on-site, we will seek immediate veterinary care for your pet at the nearest veterinarian. Owner agrees not to hold Carolina K9s responsible for any injuries or adverse reactions to grooming.
We may refuse to groom the pet or stop the grooming appointment early if we determine the pet needs immediate veterinary care. This may result from discovering open wounds, weeping eyes, nose, or ears, breathing problems, etc.
I, the pet owner, request Carolina K9s to perform basic CPR and First Aid in the event of an emergency.

Owner Responsibility
Owner agrees to be solely responsible and liable for any and all acts and behavior of their pet, including injury to the pet
itself or injury to Carolina K9s staff. The owner must disclose any known behavioral or aggression issues as well as keep Carolina K9s
updated as any such events arise. Aggression will not automatically be grounds for dismissal from grooming; however the knowledge of such tendencies is crucial to ensure our safety and the safety of your pet.
Vaccines
All pets must be up-to-date on required vaccinations. Rabies, Parvo (PV, Pv, or CPV), and Distemper are required before being groomed by Carolina K9s. DHPP, DHLPP, DA2PP-L, and DA2PP-C all include both Parvo and Distemper. Bordetella is recommended, but not required.
Payment, Cancellations, and no-shows
Owner agrees to pay the current hourly grooming rate for services rendered. You will be advised of grooming rates prior to any work being done and 30 days prior to a change in fees. The hourly rate is all inclusive unless specialty products, supplies, or equipment are required.
Payment is due at time of service. After 72 hours, a late payment fee of $5 per day will accrue. We accept cash, checks, and major credit cards, including options to pay online. There will be a $25 processing fee for all returned checks, as well as loss of check-writing privileges for future services.
Cancelled appointments and no-shows greatly affect our business. Therefore, no-shows and cancellations with less than 24 hours notice will be charged the minimum hourly rate, which must be paid prior to scheduling another appointment.
Photographs
This release authorizes Carolina K9s to take occasional photos or videos of your pet for the client file, for the company website, and/or social media pages. All photos taken are the property of Carolina K9s without compensation to the owner.
